Fees and PaymentCurrent fees - all costs included.
These fees include the cost of all medications, supplies, doctor charges, etc. If you are over 45, and don't already have an EKG on file with Dr. Bowers' office at the time of surgery, you will need to have an EKG performed at MSRH - Dr. Jimenez will bill the reading fee for this separately (about $45) Payments in full must be received no later than two weeks prior to your surgery. Dr. Bowers will send you a pre-surgery packet, normally within two months of surgery, with an exact breakdown of where and how the checks are to be written. You will also receive a checklist documenting the status of your file in terms of letters, missing information, etc. If you must cancel your surgery within 6 weeks of your surgery date, administrative fees may apply. Note: After the second cancellation, you may be required to pay in full prior to scheduling a third surgery date. We highly recommend sending payments via registered mail or courier to avoid it getting lost in the mail. Mt. San Rafael Hospital can accept credit cards for the full amount of their portion (please check with our office before making these arrangements). Dr. Bowers' office can accept credit cards for up to $2000 of that portion of payment. In regards to patients seeking insurance coverage, due to the technical complexity and physical demands inherent in performing a one-stage vaginoplasty, full payment for Dr. Bowers' portion only is expected. Insurance claims will gladly be filed for you by our staff and, if pre-approved, patients invariably receive reminbursement from their insurers. However, due to the costs associated with surgical care here in the US and the poor levels of reimbursement offered to providers, Dr. Bowers cannot accept the typical deep cuts offered by most insurers. This means that Dr. Bowers' portion of the procedure must be paid in advance. If not, there must be expectation that the difference between the insurance payment and full payment will be pre-paid by the patient, regardless of preferred provider status. The hospital portion, however, can be left unpaid if the procedure is pre-approved. Janet and Robin will gladly help you with that pre-approval process. We require a $500 Deposit in order to reserve your date of surgery, payable by personal or cashier's check to Dr. Bowers. Please contact Dr. Bowers if you need to cancel or reschedule your surgery. The deposit is non-refundable. Please use this Application Form in PDF Format or RTF Format when sending in a deposit for SRS. Note: Due to cost increases at Mt. San Rafael Hospital, we have been forced to raise our fees for primary, MTF surgery, which requires an extended stay in the hospital. However, with recent improvements in one-stage technique, we believe that most of our patients will not opt for a second-stage labiaplasty, making the inflation-adjusted costs little more than they were 20 years ago! |
